Complete your Cover-More travel insurance application below. Please read the Cover-More PDS carefully — pay particular attention to the T&Cs and any medical exceptions, and submit an EMC assessment if applicable.

Section 1 — Passengers

Full name of each passenger (as per passport) with date of birth.

Are all travellers currently living in Australia, and either Australian citizens with a valid Medicare card, or holding a visa allowing them to live, work or study in Australia?

If any traveller has already left home in Australia, a 3-day no-cover period applies from the date the policy is purchased.

Do any travellers require additional luggage cover?
Do any travellers require cover for snow skiing, snowboarding or snowmobiling?
Are any travellers travelling on an ocean or sea cruise for 2 or more nights?
Do any travellers require Adventure Activity cover?
Do any travellers require Motorcycle / Moped Riding cover?
Section 5 — Existing Medical Conditions
An Existing Medical Condition (EMC) is any disease, illness, medical condition, dental condition or physical defect that you have or have had — even if cured, controlled or in remission. Some are automatically included; others require an online medical assessment and may incur an additional premium. Please answer every question carefully.
In the last 12 months, has any traveller been hospitalised, treated in the emergency department, seen a specialist, or had day surgery?
Is any traveller taking prescription or over-the-counter medication, having regular check-ups, or under investigation / awaiting diagnosis or surgery?

e.g. insulin for diabetes, aspirin for strokes, paracetamol for back pain.

Has any traveller got a chronic, ongoing or reoccurring condition?

e.g. arthritis or back pain.

Has any traveller ever had a medical condition or required surgery involving: kidneys, liver, cancer (even in remission), joint / back / spine, brain (dementia, epilepsy, head injury, tumours), heart, stroke / clots / DVT / PE, or respiratory system (COPD, emphysema, chronic bronchitis)?
If any traveller is pregnant: are there complications with this or a previous pregnancy, is it a multiple pregnancy, or was conception medically assisted?

Pregnancy cover is limited to unexpected complications up to the 24th week. Childbirth and newborn care are not covered.

Is any traveller experiencing any signs or symptoms where a medical diagnosis has not been sought?

e.g. chest pain, shortness of breath, a persistent cough, unexplained bleeding.

Section 6 — Acknowledgement & signature

Your application is sent securely to Aussie2Italia. Any adjustments to travellers, dates or cover level may change the final Cover-More premium.
